Obtaining Documents
Each health care professional licensed by the Oklahoma State Board of Medical Licensure and Supervision has a file containing application(s), renewals and other information, including formal disciplinary action taken by the Board. Copies of any documents are available upon written request. You will be charged $0.25 per page copied. Certified copies are available at a cost of $1.00 per page. You may request to be contacted before copying if you believe the cost may be considerable to obtain an estimate of the cost.
The Board offices are open Monday through Friday from 8:00 A.M. to 4:30 P.M. You may visit the office and review the public file of any licensee. You may at that time select the entire file or any portion of the file to be copied (cost is $0.25 per page). Depending on the availability of staff, you may wait for the copies to be made or staff will contact you when they are ready.
For copies of public file and/or disciplinary action documentation you should fill out the File Review Request form on this page and mail or fax it to:
